TRIAGE
In accordance with the instruction of § 6 section 12 of the Regulation of the Minister of Health of June 27, 2019 on the hospital emergency department, as well as the maximum time referred to in § 13 section 1 and § 12 section 3 points 2 and 3 of the abovementioned regulations,I inform the following:
1. Patients qualified according to the TRIAGE procedure to the colors: blue and green may go to the unit providing primary care services from 8:00 to 16:00, and from 16:00 to 8:00 and on weekends and holidays can go to the point providing health services under the Night and Holiday Health Care located in SP ZOZ WSS No. 3 in Rybnik in pavilion No. 6.
Otherwise, the waiting time for the provision of services in the ER will be 120 minutes for green and 240 minutes for blue.
2. We would like to inform you that the maximum time for the HED doctor to make a decision:
- 24 hours to refer a Patient to another Department of the Hospital.
- refusal to admit a person who is not in a state of sudden health risk to the Emergency Department is 4 hours.
Admission to the hospital
A set of documents necessary for admission to the hospital:
1. Referral from a doctor (not required for patients in direct health or life threatening situations and for women reporting for childbirth)
2. Health insurance card - floppy disk (applies to persons belonging to the Silesian Provincial Branch of the National Health Fund),
3. Proof of insurance - documents confirming the right to health services are:
• for working people who are employees:
- certified with the stamp and signature of the employer with the current date, copies of ZUS forms (one of the three listed): ZUS ZUA, ZUS RMUA or ZUS ZZA
- certificate of application for health insurance, issued by the employer, confirmed with a seal and signature, with the current date
- insurance card currently confirmed by the payer - valid for 30 days from the date of confirmation (for 30 days after termination of employment - dismissal - the right to free benefits)
• for persons conducting business activity:
- ZUS ZUA form and confirmation of payment of contributions in the form of a bank account statement containing information on the health insurance contribution (confirmed by the owner's stamp and signature) or a postal certificate, the last proof of payment for health insurance (with the current date)
• for persons registered in the Labor Office or receiving benefits from the Social Welfare Centre:
- certificate of application for health insurance issued by the contribution payer - Poviat Labor Office or Social Welfare Centre, certified with a seal, with the current date
• for persons insured in KRUS:
- certificate of application for health insurance from KRUS (or proof of the last payment), certified with a seal, with the current date
• for old-age and disability pensioners receiving benefits from ZUS:
- the last pension or disability pension slip, if it is currently provided when the benefit is paid
- new pensioner ID card (plastic)
- decision on granting an old-age or disability pension
• for family members insured by their relatives (spouses, parents, grandparents, etc.):
- if the policyholder is an employee - in addition to the ZUS ZUA, ZUS RMUA or ZUS ZZA forms - the ZUS ZCZA or ZUS ZCNA forms, certified by the employer's stamp and signature, with the current date, or the insurance card currently confirmed by the payer - validity 30 days from the date of confirmation, with registered family members entitled to free benefits
- if the policyholder conducts business activity - a copy of the ZUS ZCZA form with the stamp of the Social Insurance Institution, certified by the policyholder, with the current date and confirmation of payment of contributions in the form of a bank account statement containing information on the health insurance contribution (confirmed by the seal and signature of the owner) or a postal certificate the last proof payments for health insurance (with the current date)
- if the insuring person is registered at the Labor Office - a certificate from the PLO with an annotation about the registration of a family member (members) for health insurance (name and surname, degree of kinship, date of birth, date of registration for health insurance)
- if the insuring person receives benefits from the Social Welfare Center - a certificate that a family member was registered for health insurance when providing it
- if the policyholder is insured in KRUS - a certificate confirming the application of a family member (members) for insurance in KRUS
- if the policyholder is a pensioner - in addition to the last retirement or disability pension or a new ZUS ID card - a certificate from ZUS with information on the registration of family members for health insurance. In addition, family members - children over 18 years of age are required to present a valid school or student ID
4. Identity card or other identity document containing the PESEL number
6. Own NIP (employed persons),
7. NIP of the workplace (employed persons),
8. Medical documentation relevant to the undertaken hospital treatment (test results
medical examinations, additional laboratory and imaging examinations, last discharge cards
from the hospital or other important health documents, if available).
In the absence of documents confirming health insurance and in the absence of a referral, when it is required, the costs of the provided health service are covered by the patient on their own.
In the event of an emergency, documents confirming health insurance may be presented at a different time, not later than within 30 days from the date of commencement of the provision of healthcare, provided that the patient is still hospitalized.
If it is not possible to present documents confirming insurance within the above-mentioned period, these documents may be presented within 7 days from the end of the provision of health care services.

Registration is located on the ground floor of the Specialist Clinic building.

- personal (required each time for the first visit to the clinic),
- by telephone (does not apply to Trauma and Orthopedic Surgery and General Surgery Clinics),
- through a third party.

• a pensioner's or pensioner's ID card or a current pension or disability pension slip

• KRUS identity card with proof of payment of the last health insurance premium

• family ID card with family members entered along with the current date and stamp

Access to the hospital and parking
The hospital can be reached by public transport buses no. 3, 6, 31, 40, 45, 46, 311A, 311B

For people coming to the hospital with their own cars, there is a paid, unguarded public parking available:
- parking up to 3h - PLN 5.00
- each subsequent started hour - PLN 2.00
and a 30-minute car park, where parking for up to 30 minutes is free and the fee for each commenced hour of parking is PLN 6.00
The following persons have the right to enter the Hospital (parking next to the Hospital Emergency Department):
- delivered to childbirth;
- in states of direct threat to health or life,
- disabled,
- authorized by the director of the hospital.
Patient Responsibilities
The patient is obliged to comply with the following rules:
- Show respect for the rights of other patients.
- Compliance with the rules of social coexistence, especially respect for dignity, intimacy, beliefs and moral principles, as well as friendly and polite attitude towards other patients, visitors and staff of the Hospital.
- Observe the principles of personal hygiene and appropriate clothing (e.g. pyjamas, tracksuit).
- Maintaining law and order in the place of stay.
- Adhere to the established organizational order in the ward, in particular staying in the room at the time of rounds, medical and nursing procedures and when serving meals.
- Observe the quiet hours between 2200 and 600 and stay in the ward at that time.
- Notifying the nurse each time of the intention to leave the ward (e.g. in order to make purchases in the hospital shop).
- Provide the staff with full information about past and current diseases and all medical aspects that may help in treatment.
- Informing the attending physician about constantly taken medications not related to the disease entity that is the reason for hospitalization. The use of these drugs during your stay in the Hospital requires the consent of that doctor.
- Follow the instructions and recommendations of doctors, nurses, physiotherapists, technicians and other medical personnel, both in the diagnostic and therapeutic process as well as in organizational and administrative matters.
- Adhere to the agreed diagnosis and treatment plan, as well as medical and dietary recommendations.
- Respect the property of the hospital.
- Observe the ban on tampering with medical, electrical, gas, ventilation and heating apparatus and devices.
- Compliance with the obligation to leave clothes (for the duration of hospitalization) in the "Clothes Deposit" and use the option of depositing valuables in the "Valuables Depository of SP ZOZ of the Regional Specialist Hospital No. 3 in Rybnik".
- Use mobile phones and other devices (e.g. laptops, MP3 players, etc.) in a way that does not create a nuisance for other patients and Hospital staff or interfere with the operation of medical apparatus and devices and outside of curfew hours. The director of the hospital may specify zones and rooms where the use of mobile phones is strictly prohibited.
- Compliance with the ban on trading in, possessing and using alcohol, drugs and intoxicants as well as smoking cigarettes on the premises of the Hospital.
- Compliance with the Order Regulations and other regulations in force at the Hospital as well as health and safety and fire protection regulations.
Responsibilities of the patient using outpatient services
The patient is obliged to comply with the following rules:
- Show respect for the rights of other patients.
- Compliance with the rules of social coexistence, especially respect for dignity, intimacy, beliefs and moral principles, as well as friendly and polite attitude towards other patients, visitors and staff of the Hospital.
- Compliance with the rules of personal hygiene and appropriate clothing.
- Maintaining law and order in the place of stay.
- Adhere to the established organizational order in the Clinic / Department.
- Provide the staff with full information about past and current diseases and all medical aspects that may help in treatment.

Footwear and protective clothing
People who want to participate in a family birth are required to report to the Emergency Room and change into protective clothing.
Clothes deposit
- The clothing deposit for patients, in which the clothing of patients hospitalized at the SPZOZ Regional Specialist Hospital No. 3 in Rybnik is stored, is located on level -1.
- Items left in the Clothes Deposit can be collected by patients or their families every day from 7.00 am to 6.00 pm. To collect clothes, please report to the Emergency Room.
- The clothes are issued on the basis of the "Confirmation of receipt of the patient's belongings" receipt, which the patient receives upon admission to the Hospital.
Clothes left in the Clothing Depository are issued to the patient on the basis of the above-mentioned receipts. They are also issued to the patient's family or to a person authorized by him upon presentation of the identity document of the receiving person.

The Independent Public Health Care Team of the Provincial Specialist Hospital No. 3 in Rybnik provides medical documentation in the manner and on the terms set out in the Act of November 6, 2008. About patient rights and the Ombudsman for Patients' Rights.
Pursuant to Art. 26 par. 1 of the above-mentioned act, a copy of the medical documentation / excerpt / certified copy is issued:
- to the patient
- the patient's statutory representative (e.g. parent, legal guardian),
- person authorized in writing by the patient.
In order to obtain a copy of the medical records / excerpt / excerpt, a written request must be submitted to the secretary's office of the appropriate Department of the Hospital / Registration / Outpatient Clinic / Hospital Information.
The application should be submitted on the form available at the above-mentioned places

Prior to launching the laboratory, the unit had measurements of X-ray dose distribution performed
surrounded by devices - based on the results, it was found that the limit dose of ionizing radiation does not exceed 1mSv/year for public spacesand 3 mSv/year for laboratory rooms outside the office.
The unit is controlled by ŚląskieState Voivodship Sanitary Inspector
in Katowice, 40-957 Katowice, ul. Raciborska 39 - Department of Radiation Hygiene.
The unit controls the exposure of workers using individual dosimetry - over the past 12 months (from 01/01/2022 to 31/12/2022) the measured effective dose was
below 0.1 mSv.
Appendix No. 4 to the Atomic Law (Journal of Laws of 2021, item 1941); ionizing radiation dose limits,
the permissible value of exposure to ionizing radiation is:
- employees: 20 mSv/year;
- total population: 1mSv/year.
Attention:
on the basis of the measured and calculated doses, it is concluded that the activityin the past 12 months has not had a negative impact on human health and the environment.
X-ray and Mammography laboratories do not release radioactive substances into the environment.
